What is refractory epilepsy?

People with epilepsy have seizures. During a seizure, the nerve cells in the brain don’t communicate like they should. The usual electrical activity in the brain changes. These seizures may last a few seconds or a few minutes.

Some people with epilepsy will have, or will one day develop, refractory epilepsy. This means that medicines don’t work well, or at all, to control the seizures.

If you have refractory epilepsy, the type of seizures you have may affect your treatment. Seizures may be:

  • Generalized onset seizures. This means they involve both sides of your brain.
  • Focal onset seizures. This means seizure activity starts in a smaller part of your brain. It may later spread out to a wider area.

Refractory epilepsy can have a big effect on your life. People with this type of epilepsy may have trouble at work or school. They may worry a lot about when their next seizure will come. They may also have injuries that result from their seizures. If your healthcare provider thinks you have refractory epilepsy, they may suggest that you visit a medical center that specializes in epilepsy.

What causes refractory epilepsy?

Some seizures have known causes, such as head injuries, infections, fevers, or brain tumors. But often the cause of seizures in epilepsy is not known.

In the same way, it is not clear why some people with epilepsy are not helped by antiseizure medicines. Or why these medicines sometimes stop working. The electrical activity in the brain during a seizure might get so high that medicines are no longer able to control it. But why this happens is not known.

What are the symptoms of refractory epilepsy?

The symptoms of a seizure can include:

  • Convulsions, or shaking movements
  • Loss of consciousness
  • Confusion
  • Tongue biting
  • Lip smacking
  • Loss of bowel or bladder control
  • Uncontrolled eye movements 
  • Staring into space
  • Falling
  • Muscle rigidness, tremors, or twitching
  • Odd behavior (such as screaming or yelling)

If you still have seizures while you’re taking medicine for them, you may have refractory epilepsy.

How is refractory epilepsy diagnosed?

Your healthcare provider will likely ask you many questions about your seizures. You will likely also have a test called an electroencephalogram (EEG). This involves placing electrodes on your scalp to measure your brain’s activity. Sometimes, the EEG can be watched for long spans of time at home or in the hospital.

Your healthcare provider may also want you to have a CT or MRI scan of your brain. Depending on the type of seizure activity and its location, you may need surgery. This would be done by a neurosurgeon who specializes in surgery for epilepsy. Your healthcare provider may do more tests like these to find out where your seizures are starting.

You will need to work closely with your healthcare provider to find out whether your epilepsy is refractory. Your provider may need to watch you closely during more seizures to try several medicines and combinations of medicines before they feel your condition is refractory. Your healthcare provider may want you to keep a daily journal of seizure activity and check in often to report your symptoms and any side effects as you try different medicine doses.

How is refractory epilepsy treated?

Your healthcare provider may suggest that you take other antiepilepsy medicine. You may take it alone or with other medicines you are already taking. There are many different medicines in this class. Some that may be tried are:

  • Carbamazepine
  • Phenytoin
  • Neurontin
  • Lamotrigine
  • Levetiracetam
  • Oxcarbazepine
  • Tiagabine
  • Topiramate
  • Zonisamide
  • Valproic acid
  • Vigabatrin
  • Clobazam

If medicines aren’t helping with your seizures, you may need:

  • Surgery. Surgery may be very helpful if you have refractory partial epilepsy and the part of the brain where the seizures start can be found and safely removed. Your healthcare provider may advise surgery if you still have seizures after trying several antiepilepsy medicines. During the procedure, a surgeon who specializes in epilepsy surgery will take out the part of your brain that’s starting the seizures.
  • Electrical stimulation. If you can’t have or don’t want brain surgery, your healthcare provider may suggest vagus nerve stimulation with an implantable device. The device is placed under your skin in your chest. Wires connect it to the vagus nerve in your neck. It sends a current to the nerve. With this device, you may have fewer seizures. It may also help lessen the severity of a seizure that has already started.
  • Ketogenic diet. A special diet that may help some people control their seizures. This type of diet is high in fats and low in carbohydrates. If you follow this diet, you will need to work closely with your healthcare provider and take supplements of certain nutrients as needed.

Can refractory epilepsy be prevented?

It might not be possible to prevent all of your seizures. But it is very important to use medicines exactly as your healthcare provider directs. Using your medicines the right way may help them work better to control your condition.

Living with refractory epilepsy

Living with refractory epilepsy can be hard. People with this health problem may worry a lot about when their next seizure will come. They may have trouble at work or school. They may no longer be allowed to drive. They may also have injuries that result from their seizures. It is important to do what you can to limit your chances of injury.

When should I call my healthcare provider?

Be sure to talk with your healthcare providers about when you should call. They will likely tell you to call if:

  • You continue to have seizures.
  • Your seizures become worse in intensity or frequency.
  • You have new symptoms during or after your seizures.

Call 911

Call 911 if you:

  • Have difficulty breathing or waking after the seizure
  • The seizure lasts longer than 5 minutes
  • You have back-to-back seizures

Key points about refractory epilepsy

  • Refractory epilepsy occurs when your antiepilepsy medicines are no longer controlling your seizures. Often the cause of refractory epilepsy is not known.
  • Your healthcare provider will likely give you other medicines to try to get your seizures under control. If this does not work, other choices may be surgery to remove the part of the brain where the seizures start or electrical stimulation of a nerve leading to the brain.

What principles define how you live?  I would like to help others and to assist me.  Lord Jesus, please, forgive me of all of my sins, transgressions & Iniquities That I have been bearing inside of my heart.  The Lord God will not hear you; if you are having Iniquities inside of your heart according to Psalm 66:18.  As far as my first principle is to Love the Lord God with all of your heart, soul, mind, and spirit.   The second is just like it.  I need to love my neighbor as myself.   Lord Jesus, please, teach me how to officially love myself and love other people for who they are in their life.  Second, I need to get my house in complete order, & to maintain my own apartment.   I need to start taking care of myself a whole lot better right now.   My second principle is to take care of myself & to continue to learn how to become responsible for making better decisions for myself.   I need to take full responsibility of my apartment right now.   Third, I need to start attending Church on a regular basis.   I had Covid-19 for almost a month.   I need to become more active with my Church and to stay out of trouble throughout my entire life right now.   My third principle is to maintain my Sobriety; Especially when it comes to alcohol and drugs.  I need to start attending my AA Meetings on a regular basis.   I need to stay healthy throughout my life.   Fourth, I need to make amends with my adversaries and family members on a regular basis.   My Fourth principle is to literally find out who is stressing me out as far as females are concerned.   Lord Jesus, please, help me to get my life in order for Your sake 🙏.  Fifth, I really need to take better care of myself physically, mentally, & emotionally.   Lord Jesus, I am taking 33 pills each day because the doctor told me that I have to take my medication each day for the rest of my entire life.  My fifth principle is to continue to lose weight, & to stay on my diet of both physical and spiritual foods for the rest of my entire life.   Sixth, I need to stay away from toxic people!  I need to get a job, so that I am able to pay my bills and rent each month.   My Sixth principle is to find a job that wouldn’t stress me out.   I need to start working with my hands again.  Therefore, I would like to pay off my credit card each month and bills that I bear inside of my apartment right now.  Seventh & Finally, I need to maintain a positive attitude throughout my life right now.   I have to start participating in Church activities in order to keep myself occupied in a positive manner.   My Seventh principle is to listen to your voice each day throughout my life.   I need to become more transparent with God, Jesus Christ, other people, and with myself.  Lord Jesus, please, enable me to accomplish these things in Jesus Name I Pray 🙏 🤲!  Amen and Amen!  Thank you for reading my essay today, & to please keep me and my entire family members and friends in your prayers.   God Bless All of You 🙏! What have you been putting off doing?  Why ?  I want to inform all of you that I was in the hospital for 5 days.  I have been putting off cleaning up my apartment because I had Covid-19 for a total of three weeks.  I also have been putting off traveling to Toledo, Ohio from attending a University of Toledo Football & Basketball game because I have been physically and mentally sick.  Also, I didn’t have any money to travel to both Toledo & Charlotte NC.   I have been putting off going to the library because I had Covid-19  for almost a month.   Please keep me and my entire family members in your prayers and thoughts.   Thank you for allowing me to answer this question.

When was the first time you really felt like a grown up (if ever)?  If you really want the truth, I was performing adult responsibilities as a 7 year old child because I had to take care of myself & my siblings.   I want to inform everyone that my mother told me to my face that I was the man of the house.   At first, I was living with my grandparents because my mother was sick at the time that I was born; because my mother was suffering from Post Parteum Depression for a little while.  Second, I want to inform everyone that I used to attend Church with my grandparents every Sunday & Thursday during the 1970s.   This had given me an opportunity to take care of other children.   I know that I was a child, but I was only a child, but I had to grow up very fast because the 1970s was a very dangerous time in my life.   I know that I was a latchkey child for a total of 7 years; From 1972 to 1979 because my mother was working at the time.  Third, I want to inform all of you that I really didn’t have an actual childhood.   I had to protect my siblings from the other children in my neighborhood because my mother was living in the Projects as well as other houses inside of the Westside of the Dayton Ohio Area.   The elementary school was only two city blocks from the apartment that my mother lived at.  The school that I used to attend was Edison Elementary School in the Dayton, Ohio Area.   From 1977 to 1978, I was bussed to an European American neighborhood school called Franklin which was located in East Dayton Ohio.  My childhood was so intense that I had to fight other kids that were in my neighborhood.  Even though I lost my fights most of the time.   I am the oldest child in my family.   I had a childhood to a certain extent because I was playing basketball for the YMCA Basketball Team during the year 1976 to 1979.  I was in the Boy Scouts Troop #19 for three years.  My mother made it possible for me to play basketball as a child.  Whereas, my grandparents made sure that I was safe & seperated from my mother’s husband because he tried to kill me on several occasions throughout my life.  I have strayed away from my own family members because taking care of my siblings became a major problem.  Although I had to do adult things, but it had affected my relationships with my siblings and other children.   I want to inform everyone that I have accomplished a lot during a 18 year period.   I had to try to avoid getting into trouble as a child because I had succumb to peer pressure by being forced to drink alcohol and smoke marijuana with my friends.   I really need a lot of prayer because there is a young lady who have caused me a great deal of distress during an 18 month period right now.   This lady is 30 years younger than me.   I am very sorry for going off on a tangent today because I have had a total of 3 seizures this year. I have to really stay calm & relaxed under pressure.   My therapist is helping me to literally cope with my problems with my adversaries & other women who I have a problem with right now.   Currently, I have learned how to become honest and content with myself.  I am more mature right now because I have been clean & sober for 34 years now.   I have went through some sort of an experience that is very hard to describe right now.  I have a passion for helping women and children because my own biological father was not involved in my life, and it really bothers me because everytime I look at my own birth certificate; the biological fathers information is blank.  It always makes me feel that I am an illegitimate child.  I have more faith in God and Jesus Christ right now.   I thank God for saving me from myself and from my adversaries right now.  Growing up in the Dayton Ohio Area has made me feel like I am walking on eggshells right now.  I will admit that I have a profound fear of women because whenever they start yelling at me, I have to take a long walk because I like to preserve my sanity.  I had to learn how to listen to women because they can be compassionate and understanding.   I had to learn all by myself because I didn’t have an actual biological father in my life, and it affects me personally.  Lord God, please forgive me of all of my sins, transgressions, and Iniquities because I prefer to carry the Love that I have for God and Jesus Christ as well as His Entire Creation Forever.  My AA Sponsor helped me to learn how to forgive other people for hurting me.   It is that I hope and pray that this young lady can forgive me in the same way I have forgiven her.  Therefore, I have changed my life for the better.   Frankly, it is the same little girl that I have hurted her, and I feel very remorseful and sad because she has grew up right now and she is very angry at me.  If I see her; I would like to continue to pray for her personally.   If she yells at me, I will accept it because everyone is entitled to a freedom of expression.   What I actually have to do is search for her and console her for the rest of her life.  I know that she is very angry at me. I only wish that I can find her.  I will tell her family members that I am very sorry for injuring her. Lord Jesus, please, give her whatever she deserve in a positive manner.  I still have to make direct amends with everyone in my family as well as my adversaries right now.   If it is the Lord’s Will, I would like to make amends before I die.  Lord Jesus, please show me how to make complete amends with very difficult people. I have learned how to accept constructive criticism & excessive vulgarity from the people who I have done wrong to in my life. I am still remorseful and I forgive her and her family for the wrong that I have done. Lord Jesus, only You know exactly who she is personally.
